Counseling Agreement

 Counseling is a process whereby a counselee seeks to resolve personal or interpersonal issues with the assistance of another trained and caring individual. As your counselor I will bring to the sessions my knowledge and experience; but the ultimate responsibility for growth and change rests with you the client(s). You are invited at any time to ask questions about my methods, and/or the direction the counseling is headed.  My approach is one of teamwork, and therefore it is important that you take an active role in the determination of goals, the approach and process used to reach those goals.

Sessions normally last 50-60 minutes. You are not obligated to complete a specified number of sessions. You are required to pay for completed sessions the day of the session. The 55-60 minute session rate is $65.

If you need to cancel an appointment, I appreciate being notified at least 6 hours before the appointment.  I reserve the option to charge one half of the session fee if you do not give notification. I/We, the undersigned client(s) understand and agree to abide by the above policies.
